GEUS wants to collect and learn from incidents that could have had severe consequences, but where someone got away with the scare. We therefore encourage all who have experienced situations where defective equipment, lack of training, negligence, stress, fatigue or any other reason, gave rise to a near miss incident.

GEUS also wishes to develop a culture where we talk openly about near misses, even if they were caused by wrong judgement of a situation. Thus, as a principle, we ask for names of the project, the P.I. and the notifier of the incident. However, one is allowed to anonymise a notification of a near miss incident.
